You MUST mail or deliver forms and payment to Dale City Animal Hospital on or before the Wednesday prior to class
start. Make sure you read the class policy as well! Make all checks or money orders out to Karen Peak.
Forms and payment not in by the cutoff may result in the inability to get into the desired class. PLEASE allow 5 business days for mail delivery.
